Frenuloplasty in Pune: Treating a Short or Tight Frenulum
Frenuloplasty is a short operation to lengthen a tight or short penile frenulum (frenulum breve), the band of skin on the underside of the penis that joins the foreskin to the head. The band is cut across and stitched lengthwise, so it no longer pulls or tears during erections or intercourse. It is usually day care under local anaesthesia.

What is a short frenulum?
The frenulum is a small band of elastic skin on the underside of the penis that joins the foreskin to the head (glans). In some men it is short or tight, a condition called frenulum breve. During an erection it pulls, causing pain, a downward bend of the head, or a tear that bleeds. Each tear heals with scar tissue that can make the band even tighter, so the problem often keeps returning.
Many men put off seeking help out of embarrassment. A short frenulum is common, and treatment is simple.
Who may benefit, and how is it assessed?
Frenuloplasty may help if you have:
- pain or pulling during erections or intercourse;
- repeated tearing or bleeding of the frenulum;
- difficulty pulling the foreskin fully back because of the frenulum;
- a head that bends down when erect because of the band.
The doctor examines the frenulum and foreskin in private. They check whether the foreskin itself is tight (phimosis), look for infection or skin conditions, and ask about diabetes and medicines. Mild cases can sometimes improve with gentle stretching and a steroid cream.
What are the options?
| Option | What it can do | Limitations |
|---|---|---|
| Steroid cream and gentle stretching | May loosen a mildly tight band | Slow; often not enough when there is scarring |
| Frenuloplasty | Lengthens the band and keeps the foreskin | Small chance the band tightens again |
| Frenulectomy | Removes the band completely | A slightly larger wound |
| Circumcision | Removes the foreskin and frenulum; also treats phimosis | A larger operation |
How is frenuloplasty performed?
- Local anaesthetic: a small injection numbs the penis.
- Release: the tight band is cut across (transversely) with fine instruments or cautery, so the skin opens into a diamond shape and the tension is released.
- Lengthening: the diamond is stitched closed lengthwise (longitudinally), which makes the frenulum longer. A V-Y design is sometimes used.
- Closure: fine dissolvable stitches close the skin, and a light dressing is applied.
The video on this page explains the condition and the treatment.
How should you prepare, and what should you ask?
- Tell the doctor about diabetes, blood thinners and any skin conditions of the penis.
- Wear loose cotton underwear on the day.
- Plan a quiet day or two afterwards.
Questions to ask:
- Is the problem only the frenulum, or is my foreskin tight as well?
- Would frenuloplasty or circumcision suit me better?
- When can I return to exercise and intercourse?
What is recovery like?
| Time | What to expect for most patients |
|---|---|
| Same day | Home after the procedure; mild soreness when the anaesthetic wears off |
| Days 1-3 | Swelling and tenderness; keep the area clean and dry; loose underwear |
| Weeks 1-3 | Stitches dissolve; gentle washing; night-time erections may feel uncomfortable at first |
| Weeks 4-6 | Usually cleared for intercourse and full activity |
What are the risks of frenuloplasty?
- Swelling and bruising, usually settling within days.
- Bleeding, usually stopped with gentle pressure.
- Infection, uncommon.
- The band tightening again, sometimes needing a further procedure or circumcision.
- Changes in sensation, uncommon.
Contact the clinic for bleeding that does not stop with pressure, increasing pain or swelling, fever or difficulty passing urine.
What results can you expect?
Most men no longer have pain, pulling or tearing once healed, and the foreskin is kept. Published experience reports success in most men, with a small number needing further treatment.
What affects the cost of frenuloplasty in Pune?
- Technique: frenuloplasty, frenulectomy or combined circumcision.
- Anaesthesia and facility.
- Medicines and follow-up visits.
Questions about frenuloplasty
What are the symptoms of a short frenulum?
Pain or a pulling feeling during erections or intercourse, the head of the penis bending downwards when erect, difficulty pulling the foreskin fully back, and small tears that bleed, sometimes repeatedly.
Is frenuloplasty better than circumcision?
For a short frenulum alone, frenuloplasty keeps the foreskin and is a smaller operation. A 12-year review found it successful in most men and often avoided the need for circumcision. If the foreskin itself is tight (phimosis), circumcision may be advised instead or as well.
Is frenuloplasty painful?
The area is numbed with local anaesthetic, so the operation is not felt. Afterwards there is soreness for a few days, eased with simple painkillers and loose underwear.
Will it affect sensation?
The aim is to release tension while keeping the tissue. Most men report no loss of sensation, and many find erections and intercourse more comfortable. Changes in sensation are uncommon.
What if my frenulum tore during intercourse?
Firm pressure with a clean cloth usually stops the bleeding. A torn frenulum often heals on its own but may tear again as the scar tightens. If tearing keeps happening, frenuloplasty is often advised.
Is this the same as tongue tie surgery?
No. The word frenuloplasty is also used for releasing a tight band under the tongue. That operation is described on the tongue tie surgery page.
